For me it is an extension and a twist. I was taught to get used to straightening your leg first and then start moving your body away from the pole.

First, make certain that your basic butterfly is strong and you are able to extend away from the pole, pushing your torso out so that your chest and shoulders are back and in good aligment.
    Here is what I mean:

The rest is as Chemmie says…straighten your hooked leg first and turn out slightly into the pose. One key is to keep that hooked foot pointed hard. The pointed toe will create a "hook" at the base of the heel to help hold yourself on the pole. If you dont point, your heel is more squared off rather than hooked. Turning out slightly positions that hooked heel for better hold.
