What went wrong here?

Anyone care to weigh in as to what went wrong and how this could be prevented? Was the top pole not screwed in or attached correctly?
I'm not really interested in a debate on the differences in stage manufacturers, as this has been discussed at length on other threads.

It's difficult to tell for certain but it looks like this is the PS Star Standalone stage.
If so, it may be impossible to avoid that debate because this pole was not set up incorrectly.
This is how the original PS Star Standalone was made. The top pole unscrews and can come apart during use. The newer version of this pole no longer screws together.

Yes it does look like the PS Star Stage because of the white platform, but not for sure. Although I can say, I have yet to see a "fake" or "counterfeit" stage pole, and this one does not look home made.
@Mary Ellyn….can a gap not be felt or seen if the top pole is unscrewing? I understand this is not safe and would not be acceptable if I had a pole that came unscrewed on it's own. It just looked like the girl placed her hands about where the pole "broke".
Also if you look real closely right when she places her top hand on the pole, she moves it back and forth slightly, as if she"s checking it or something?

It looks like a star standalone. The old version had skrew fittings. They have since changed their fittings.

I might be wrong on this, but I think PS presses the threads into the screw pieces, so the threads light have slipped out?

WOW! That's a PS Star Stand Alone alright! The top piece unscrewed. I had that happen to me and thank fully I was able to slide over it come down, regrip, come up and fix it.
The thing is that the threading on my stage was never really tight – I complained about it and finally got a new piece but it was the same thing all over again. You think it's tight enough – but it's not. Because they screwed together if you turned or spun down the pole in the opposite direction of the threading it wouldl come loose and you would have no real way of knowing how bad it is until you feel it turn. i hope this girl was ok.
BTW – I am told that they fixed the threading issue so this is likely an older model – however they never offered the safe upgrade to anyone who was sold defective poles. That's why I switched to X-pole stages. I felt I deserved a safe pole and no more expense than I had put in to the pole and they disagreed.

Aly…I don't have experience with it coming unscrewed but according to what Charley wrote apparently it can happen pretty quickly without being able to see it until it's too late.
BTW…I found another copy of this video and apparently this happened at an event in Chicago.

I heard a story awhile back from a pro who said that she did a trick and the pole came undone and the top of the pole was just in her hand. Very dangerous. I would love to spread this video around as to why these aren't safe poles but it would start a bunch of crap that doesn't need to be brought up again. Also, I haven't been on the improved model (making it totally unfair for me to say anything) – I did hear better things about the current version available however I will only deal with X-Stages from now on. That's just a personal choice.

I am amazed she got back on it.. or you Charley. You could tell she already knew what had happened meaning it likely wasn't the 1st time.
